All REGISTRATIONS must be completed by June 21st. Late REGISTRATIONS will be accepted up through the start of Summer Music camp for an additional $15 fee.

Please complete ONE registration form per family member attending, however submit only one check.
Please note you will pay for the cost of the entire session, no matter how many days you attend
Once in attendance, there will be no refunds
You will be contacted at a later date regarding your ensemble placement
ALL CHECKS MUST BE MADE OUT to: WESTVIEW FOUNDATION

- Individual Student $150 - 2 Family Members $200 - 3 or more family members $250
